Advanced Clinical Practitioner - The Smithy Surgery Part-time Advanced Clinical Practitioner £54,000 to £65,250, yearly salary increments, NHS Pension provider. gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England. We have a fantastic opportunity for an Advanced Clinical Practitioner to work in our friendly practice, The Smithy Surgery based in Hyde. This role is 18 hours per week, hours of work to be worked across 3 days, Monday to Wednesday. You will be supported on site by our long-standing GP Partner Dr Metcalfe and other Salaried GPs and the Nursing & Admin Teams. Whilst working as an Advanced Clinical Practitioner, you will work alongside a supportive multidisciplinary team that includes Nursing, Allied Health Professionals, and GPs, who have a real commitment to continuous improvement, shared learning, and best practice, while being passionate about providing patients with the highest possible quality care. Main duties of the job Using EMIS, you will consult via telephone, video, or face to face, according to patient needs. (Full training and support for remote consultations will be available if required). You will be supported by the non-clinical team who have a good knowledge of our extensive local hospital and community services. The clinical team includes GP's, Practice Nurse, Pharmacist, Paramedic, and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model, and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives/ways of working and as such promote health improvement and disease prevention. In addition, you will assist to further improve standards of care and achieve QoF targets as well as KLOE objectives, as outlined by CQC. You will have full access to our gtd learning academy and clinical forums, including the NMP monthly forum, which is available to all our staff. This provides a great opportunity to grow and expand your clinician skills/knowledge and to excel as an ACP. Job responsibilities Clinical Practice Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination, and appropriate use of investigations to confirm diagnosis. Plan, implement, and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate healthcare service. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care. Refer and discharge safely ensuring that safety-netting advice is offered. Triage telephone referrals, calls from external clinicians, patients, and others into a range of services appropriately as required. Communication Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, and preferred ways of communicating. Anticipate barriers to communication and take action to improve communication. Maintain effective communication within the organisation and with external stakeholders. Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate. Non-Medical Prescribing If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes. You must maintain your knowledge and skills and prescribe within your clinical competencies. The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance. Delivering a Quality Service Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time. Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures.
