Advanced Practice Nurse, Family; Primary Care

The Centers seeks an Adult or Family Nurse Practitioner to meet the needs of our patients and clients at our Midtown Health Center. The ideal candidate will have experience working with clients who are experiencing severe and persistent mental illness and work collaboratively with our psychiatry, counseling and case management teams.
Job Duties
Provides direct patient care to patients; medical histories, examinations, and assessments. Diagnoses, treatments, educate, counsels, refer, and follow up with patients. Provides clinical services in women's health care, general medical, adult primary care, and the early intervention program. Follows approved standing orders and protocols. Works collaboratively with the director of primary care, physician(s) and other practitioner(s). Serves as an instructor, role model, and resource for student practitioners and medical assistant students. Assures compliance with standards of care and protocols and monitors care through quality assurance improvements. Conducts training sessions for volunteers and staff. Contributes to departmental and organizational decision making through active participation. Works effectively and positively with co-workers and volunteers. Other duties, as assigned by supervisor.
Qualifications
* Two or more years clinical work experience as an APN
* Master of Science in Nursing
* ANCC/AANP Certification
* Certificate of Prescriptive Authority
* Board licensed and certified in Adult NP or Family NP
* Valid Ohio Certificate of Authority for APN
* CPR and First Aid Certification
* Excellent communication skills
* Ability to lift up to 25 pounds
