Job Description & Requirements Specialty: Home Health Discipline: LPN / LVN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel LPN (Home Health & Hospice) Schedule: Mon–Fri | 8:00 AM – 4:30 PM | No Call Shift: Days Experience: 1 year Summary: Provide direct patient care under physician orders and RN guidance, following Oklahoma Board of Nursing standards. Responsibilities include assessments, documentation, IV therapy (per policy), wound/ostomy care, lab draws, drainage procedures, patient education, and home-based skilled nursing care as assigned. Requirements: Valid Oklahoma or Multi-State LPN License Current CPR Certification Practical Nursing diploma/certificate Skills: Strong communication, organization, attention to detail, and knowledge of disease management and clinical pathways. Work Setting: Patient-facing care, collaboration with healthcare teams, home visits, quality and performance participation. Short, steady weekday schedule with no call.
