Overview:

POSITION SUMMARY

This position provides services at the TCRHCC main campus. Pharmacist is responsible for providing a comprehensive pharmacy and medication management service to patients and ensures a consistent level of service in accordance to TCRHCC policies. Direct patient care and services are provided to neo-natal, pediatric, adolescent, adult, geriatric, oncology, and obstetric patients. This position requires working rotational shifts: day, evening, nights, weekends, and Holidays. The position provides the opportunity for career advancement through tier levels of Pharmacist to Advanced Practice Pharmacist as determined by pharmacy experience. Advanced Practice Pharmacist serves as a clinical pharmacist for direct clinical pharmacy services for inpatient and outpatient departments. In addition to the responsibilities of Advanced Practice Pharmacist I, Advanced Practice Pharmacist II is responsible for all specialized pharmacybased clinics and providing an ongoing analysis of clinical services to develop programs that commensurate with the highest quality of care. Responsibilities also include the administration of policies and procedures and directing programs and activities in accordance to TCRHCCs mission, vision, and values. Incumbent serves as the Clinic Manager of Operations at the designated clinic(s) and provides technical and administrative supervision to professional and non-professional employees, i.e. Advanced Practice I, Staff Pharmacists, Pharmacy Technicians, Pharmacy Assistants and Pharmacy Billers.

Qualifications:

NECESSARY QUALIFICATIONS

Education (Pharmacist I/II & Advanced Practice I/II):

Doctor of Pharmacy or a Bachelor of Pharmacy degree from a school recognized by the American Council of Pharmaceutical Education

Certification (Pharmacist I/II & Advanced Practice I/II):

  • Must have a current valid and unrestricted license to practice pharmacy in a State or Territory of the United States, or the District of Columbia
  • Must have and maintain a current valid certification of the following throughout employment:
    • Basic Life Support (BLS) from the American Heart Association

Experience:

  • Pharmacist I: Not required for entry level incumbents
  • Pharmacist II: A minimum of one (1) year experience as a Registered Pharmacist
  • Advanced Practice Pharmacist I: 1) A minimum of two (2) years experience as a Registered Pharmacist; and 2) A minimum of one (1) year experience as a Clinical Pharmacist
  • Advanced Practice Pharmacist II: 1) A minimum of five (5) years experience as a Registered Pharmacist and 2) a minimum of two (2) years experience as a Clinical Pharmacist

Credentialing (Advanced Practice I/II):

Must become a credentialed member of the TCRHCC Pharmacy Staff

Other Skills and Abilities (Pharmacist I/II & Advanced Practice I/II):

MENTAL AND PHYSICAL EFFORT

Physical:

Frequent sitting, standing, walking, twisting, and maintaining balance required. Occasionally reaching, bending, stooping, climbing, kneeling, and crouching required. Incumbent must be able to do moderate lifting of up to 10 pounds for prolonged periods of time and frequently be able to push/pull up to 10 pounds in the performance of assigned duties. Sensory requirements required includes prolonged ability to see near and have colored vision. Incumbent will also need to frequently be able to have far vision, depth perception, see in fine details, hearing normal speech, and, be able to use a telephone. Must be able to hear overhead pages announced over a loudspeaker. Incumbent must be able to use both hands to do simple grasping and keyboarding. Must frequently be able to perform firm grasping and fine motor manipulations with both hands. This position does require working frequently around environment exposure of chemical agents; dust, fumes, gases; hazardous or moving equipment; and loud noises. Incumbent will occasionally be exposed to infectious disease and extremes in temperature or humidity.

Must carry out daily duties and project assignments in an independent manner utilizing knowledge and experience of the section time limits, procedures, and objectives to establish individual work priorities; and the ability to communicate information and ideas in speaking/writing, so others understand. Incumbent must be able demonstrate a prolonged ability to concentrate, adapt to shift work, and work in areas that are close and crowded. There will be frequent times when the incumbent must be able to handle a high degree of flexibility; handle multiple priorities in stressful situations; work alone; demonstrate high degree of patience; manage altercations; make decisions under high pressure; and, cope with high level of stress. Responsibilities:

ESSENTIAL FUNCTIONS:

Clinical:

  1. Conducts authorized outpatient pharmacy clinics for examination, diagnosis, and treatment of specialized clinic patients. Obtains relevant health history, follows, and adjusts the developed treatment plan, and monitors the patients outcome to the prescribed treatment plan.
  2. Reviews inpatient and outpatient medical records for safety, appropriateness, and effectiveness of drug therapy with regard patient-specific factors (i.e. age, organ function, disease state). Consults with Medical Staff when concerns arise and collaborates with nursing staff to evaluate medication use.
  3. Reviews, assesses and inputs chemotherapy treatment plans for oncology patients, verifies medication orders to ensure accuracy of dosage, treatment schedule, route of administration and safe administration based on laboratory information and evidence-based practices.
  4. Counsels patients, their representatives, and/or patient family members on safe and effective use of, including compliance with, prescribed medications, utilizing effective customer service skills.
  5. Participates in pandemic and/or disaster drills that TCRHCC has elected to take part in and including responding to actual disaster events.
  6. Develops hospital staff's pharmacological knowledge by participating in clinical programs, training pharmacy staff, students, interns, externs, residents, and health care professionals.
  7. Maintains pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies for continuing education (C.E.) required for specialized pharmacy-based clinics.
  8. Provides pharmacological information by answering questions and requests of health care professionals, consults with providers on the significance of properties of drugs regarding absorption, metabolism and excretion in all age groups.
  9. Inpatient Pharmacy Setting: a) Attends multi-disciplinary rounds to provide pharmacy-related input for patients therapeutic plans. Follows up on recommendations made and adjusts as necessary. b) Responds to Code Blue and Level 1 traumas, provides support for dose calculation, rates of infusion, routes of administration, and mixing of emergent IV/IM/IO preparations. c) Plans, reviews, and maintains emergency drugs for emergency situations. d) Documents all clinical activities, update problem lists, and update medication allergy information in the medical records. e) Provide pharmaceutical services to focus on therapeutic drug monitoring. f) Analyze medication regimens to ensure the most effective, least toxic, and most economical treatment plan to enforce and enhance the role of Pharmacy as an active member of the health care team based on current clinical knowledge.
  10. Ensure proper PPE is work at all times while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher (if available), and eye or face shield.
  11. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates)
  12. Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction.
  13. Gains competency in utilization of pharmacy softwares
  14. Supervises and guides pharmacy technicians in the delivery and execution of pharmacy services
  15. Is efficient in performance of duties and meets key performance indicators (KPIs)
  16. All other duties as assigned by the Director of Pharmacy Services, Pharmacy managers and Quality Assurance Pharmacist.

Performance Improvement:

  1. Identifies and reports any medication errors or adverse drug reactions as per the Medication Error and Adverse Drug Reaction reporting policies. Identifies areas in TCRHCC pharmacy or hospital practice that could be improved so as to minimize patient safety risks and provide more efficient operations.
  2. Selects, procures, prepares, and dispenses only those medications that conform to standards of quality for potency, purity, and effectiveness at the time they are administered or dispensed to patients. Evaluates all prescriptions for appropriateness to the age, weight and gender of patient.
  3. Demonstrates knowledge of and competency in hazardous medication management.
  4. Safely handles and prepares compounds such as chemotherapeutic agents, biologicals, or other medications; packages, manufactures, and compounds products, including unit doses and intravenous additives (including chemotherapy), that are not commercially available as a premix.
  5. Maintains necessary legal records on controlled substance drugs and other prescription products.
  6. Serves on various hospital-wide committees as a representative of the Pharmacy department. Implements the drug Utilization Review Reports and presents the review to the quality assurance committee through the Pharmacy and Therapeutic Committee as needed.

APPLIES TO ADVANCED PRACTICE PHARMACIST I/II

  1. Performs, as required or as scheduling permits, all Clinical and Performance Improvement duties listed above.
  2. Utilizes the relative health history and performs limited physical assessment related to the specialized pharmacy-managed clinics; distinguishes between normal and abnormal findings to recognize early stages of serious physical, emotional and mental problems.
  3. Medications may be monitored, titrated and/or altered per specialized pharmacy clinic guidelines; therefore, prescriptions may be initiated, canceled, renewed and/or written to adjust doses. May order and interpret laboratory tests as appropriate for specialized pharmacy clinics and monitoring inpatient pharmacotherapy.
  4. Participates, assesses, plans and implements interventions in pharmacy-managed clinics, in accordance with departmental and facility-wide policies and with the standards of professional pharmacy practice.

Managerial/Supervisory:

  1. Implements, coordinates, and supervises all clinical service-related activities within the designated Pharmacy Services area. Fosters the interest of the staff in clinical service activities and monitors the established specialized pharmacy-based clinics.
  2. Formulates clinical service policies and procedures, interpreting pharmacy administrative policies, directives, and procedures to employees, patients, and pharmacy administration.
  3. Develop, implement and establish new specialized pharmacy-based clinics as needed to provide clinical services for the service population.
  4. Develops and implements clinical services quality assurance activities using predetermined established The Joint Commission (TJC), and Centers for Medicare and Medicaid Services (CMS) standards. Works with established performance improvement (PI) committee groups to coordinate PI initiatives including resolution of identified problem areas.
  5. In conjunction with the Director of Pharmacy Services, assists in managing costs affecting the specialized pharmacy-managed clinics.
