JOB SUMMARY

The registered nurse plans, organizes, and directs home care services, focusing on community health education and experience. In this role, you will collaborate with patients, representatives, and caregivers to develop and implement individualized care plans. You will provide preventative and rehabilitative nursing procedures, offer counseling and education to patients and caregivers, and maintain accurate clinical notes. Additionally, you will communicate with physicians and other healthcare practitioners to ensure the best possible care for our patients.

DUTIES & RESPONSIBILITIES

Patient Care
  • Develop and initiate the plan of care, collaborating with patients, representatives, and caregivers
  • Provide ordered services as indicated in the plan of care
  • Offer counseling and education to patients and caregivers
  • Prepare clinical notes
  • Communicate with physicians and healthcare practitioners related to the current home health plan of care
  • Identify discharge planning needs and implement prior to patient discharge
Communication
  • Prepare clinical notes and update the primary physician when necessary
  • Communicate with physicians regarding patient needs and report any changes in condition
  • Coordinate care plan with community health related persons
Additional Duties
  • Participate in quality assessment and performance improvement program
  • Participate in on-call duties as defined by the on-call policy
  • Ensure availability of necessary equipment and services
  • Instruct, supervise, and evaluate home health aide care provided every two weeks
  • Supervise LPNs/LVNs and/or HHAs/CNAs, as directed

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
  • Graduated from an approved school of nursing and licensed in the state of practice
  • Minimum of two years of nursing experience, with preference given to those with public health or home care nursing experience
  • Current CPR certification
  • Licensed driver with an insured automobile in good working order
  • Self-directed and able to work with minimal supervision
  • Excellent observation, problem-solving, and communication skills
  • Ability to organize and prioritize workload independently
  • No management experience required, but responsible for supervising home health aides
  • Physical ability to perform required nursing skills
