Our client is a growing healthcare organization offering comprehensive benefits, generous paid time off, and a strong commitment to work-life balance The Position The Nurse Practitioner is a vital member of an interdisciplinary healthcare team, contributing to the planning, implementation, and evaluation of individualized care plans. This role provides comprehensive primary care to older adult participants in a variety of settings, including the clinic, patients homes, and affiliated long-term care facilities. The Nurse Practitioner collaborates closely with physicians and the broader care team to ensure the highest quality of care, focusing on both acute and chronic conditions. Salary and Benefits Salary Range: $106,000- $149,000, Based on Experience Health Insurance: Employer pays 75% of premium Vision and Dental Insurance 26 Days Combined Paid Time Off: 15 Vacation Days 10 Sick Days 1 Personal Day 13 Paid Holidays Mileage Reimbursement 403B Retirement Plan Long and Short Term Disability Life Insurance Flexible Spending Account Plans Legal Assistance 3 Days of Continuing Medical Education (CME) With $3,000 Reimbursement Reimbursement For Licensing Renewal Responsibilities Monday Friday, 8:00 AM 4:00 PM Evaluates participants for episodic needs, acute illnesses, and provides follow-up care following hospitalizations. Visits nursing facilities to address acute or chronic conditions when participants are unable to visit the clinic. Conducts home visits for participants who are homebound due to acute or chronic health concerns. Completes initial assessments for all new participants and conducts quarterly evaluations throughout their first year of enrollment. Limited Local Travel Oversee Care and Treatment Plans Collaborate as part of the Interdisciplinary Team Supervise and Mentor Clinical Staff Participate in Phone-Only Call Rotation (1 in 15 weeks) See patients per day Requirements Licensed Nurse Practitioner in the state of Massachusetts Certified as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) 1+ Year of Recent Experience as a Nurse Practitioner is Required Experience in primary care or geriatrics preferred Current DEA registration in Massachusetts Current CPR Certification COVID-19 Vaccination Required or Exemption About Us Titan Placement Group is a permanent placement healthcare recruiting firm dedicated to connecting healthcare organizations with high-quality candidates.
