The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.****The value you’ll bring to the team:**+ Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to thepatient from first contact and throughout the continuum of care.+ Guide patients on treatment plans, next steps in appointments (i.e. surgical prep, imaging, labs,tests, which provider to see next, etc.)+ Assist with scheduling follow-up appointments and provide information and reminders to patients& physicians’ offices.+ Ensures an extraordinary customer experience.Qualifications**The expertise and experiences you’ll need to succeed:**+ **_A graduate of an accredited school of nursing or degree program_**+ **_Three years of experience required, with one year in specialty._**+ **_Strong psychosocial assessment and clinical skills_**+ **Current Colorado RN License or a Compact RN License from a NCLA Compact State**+ **BLS certification credentialed from the American Heart Association (AHA)**This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
