Job Description:
The purpose of the Hospitalist Care Coordinator is to improve the quality and safety of the care delivered to patients during acute care episodes, as well as ensuring a smooth transition to post-acute care providers. The ideal candidate will be someone with an idealistic view of the importance of a team-based approach to healthcare delivery with a focus on improving the process to ensure better outcomes for our patients. They should have nursing experience with working knowledge of both the inpatient and outpatient care delivery systems. The expectation is that over time this position will evolve as we figure out what works, what doesn't, and what can be done better
Required Licenses and Certifications:
- Registered with Kentucky Board of Nursing or compact states
- CPR certification required
- Graduate of an accredited RN program
- Bachelor's degree (BSN) preferred
- Minimum of six (6) months' recent experience as a professional nurse preferred but not required
- Drug Screen
- Tuberculosis Test
- Background Check
- Physical Exam
- Respirator Fit
