Who We Are:

Hometown Home Health is a full service home health care agency. We offer a wide range of skilled medical home care services and clinical nursing care in the Greater Jacksonville area.

What We're Looking For:

Our team is searching for a Registered Nurse in the state of Florida who prioritizes patient care and is able to commute within the St. John's County area. This professional would provide skilled nursing care service visits to patients, including comprehensive health and psychosocial evaluation, and monitoring of the patient's condition in their homes.

What You'll Do:
  • Coordinate patient care in accordance with the treatment plan created by the physician.
  • Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
  • Participate in the patient's discharge planning process.
  • Prepare progress notes following visits for patients' charts.
What You Need:
  • Must have a nursing degree from an accredited program
  • Licensed as a registered nurse within the state of Florida
  • Adherence to federal, state, and accreditation requirements, including Medicare and Medicaid regulations
  • CPR certification required (Online certification not accepted)
  • At least one year of experience at a home health agency or within a hospital
