Description

POSITION OVERVIEW: The Community-Based Clinical Coordinator is responsible for assisting in the QMHP oversight for consumers served at MHA. This includes completion of intakes and other diagnostic evaluations as needed in collaboration with hospital discharges, and outpatient and case management needs. The Community-Based Clinical Coordinator will work with the Director to ensure that the needs of MHA consumers are met during transitions in care, both external and internal.

ESSENTIAL POSITION RESPONSIBILITIES :

  1. Collaborates with the Director for daily review of hospitalization data, communication with treatment teams, triage of priority cases, and completion of intakes in collaboration with hospital discharges and outpatient needs.
  2. Provides contact to individuals in community-based services who have recently been discharged from hospitals and emergency rooms in accordance with CCBHC requirements.
  3. Serves as point of contact for individuals served in OCK whose cases are being reviewed with interdisciplinary team, updating progress as known, and facilitating feedback into actionable items with services.
  4. Serves as a liaison with MCOs regarding case coordination and optimization of care.
  5. Provides short-term crisis or solutions-based therapy for individuals served in community settings to best meet the needs of individuals under MHA's care.
  6. Assists with QMHP oversight with treatment plans, reviewing existing treatment plans of case management team in determining when treatment plan goals are met and facilitate successful termination from services, and review of treatment progress during interdisciplinary team meetings.
  7. Completes peer reviews monitoring for appropriate utilization, services, diagnosis, and documentation.
OTHER POSITIONREQUIREMENTS :
  1. Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed.
  2. Exhibits appropriate level of technical knowledge for the position.
  3. Produces quantity of work necessary to meet job requirements.
  4. Works well with a team, keeps others informed of information needed. Treats others with respect, maintaining a spirit of cooperation.

  6. Demonstrates the ability and willingness to handle new assignments, changes in procedures and business requirements. Identifies what needs to be done and takes appropriate action.
  7. Completes assigned work, meets deadlines without reminders/follow-up from supervisor or others.
  8. Performs work conscientiously with a high degree of accuracy.

POSITION REQUIREMENTS :

The Community Based Clinical Coordinator is expected to have completed a Master's degree in social work, marriage and family therapy, psychology, or professional counseling and have a license with the BSRB. Preferred areas include knowledge of community resources, experience working in the community with youth who have Serious Emotional Disturbance (SED), ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with consumers, constituents and staff. Supervisory experience preferred. Must maintain current licensure as required for position. A valid Kansas driver's license and access to personal vehicle required.
