Are you living your life's work?Summary:Responsible for providing optimal nutrition care to patients, conducting and evaluating nutritional assessments, providing nutrition education to patients, family and staff, participation in multidisciplinary treatment team meetings.Responsibilities: 1. Demonstrate competency in the knowledge, skills and attitudes necessary to meet the nutritional needs of the program's patient population 2. Conduct thorough and timely patients' nutritional assessment and reassessment, evaluate each patient's nutritional status, degree of nutritional risk, and potential problems 3. Provide age appropriate care as indicated by the knowledge of the principle of growth and development over the lifespan, and the ability to assess and interpret data, relative to the patient's age 4. Make recommendations and determines plan of care as to provide optimal nutrition 5. Provide nutritional education to patients and families 6. Monitor patient's weight, meals and supplements as needed 7. Conduct individual, group and family therapy sessions within scope of practice 8. Provides nutritional consults as needed 9. Document each treatment session in accordance with Princeton House documentation standards 10. Complete assessment evaluation 11. Coordinate the development and implementation of the multidisciplinary treatment plan and complete treatment plan reviews within established time frames 12. Attend daily treatment rounds, providing input to and being receptive to input from other members of the treatment team 13. Identify changes in the patients' clinical and nutritional status and communicate to the appropriate member of the treatment team 14. Participate in hospital and performance improvement initiatives and goals (e.g., completion of patient satisfaction surveys, positive patient satisfaction scores, participate in outcome measurementsEducation: Education o Completion of four-year degree with B.S. or B.A. in Human Nutrition. •Related Work Experience o Two to three years previous experience as Clinical Nutritionist. •Can additional experience substitute for education? If "yes", specify type of experience (i.e. supervisory or nonsupervisory, etc) and identify how many years of additional experience:Licensure, Certification or Registration Requirements: •R.D completion or pending.
