MDS Assessor

The Grove at Valhalla Rehabilitation & Nursing Center is looking for a talented and hard-working MDS Assessor to join our ever-growing team. We are seeking qualified candidates who have experience as an RN and are committed to help our patients and facilities receive the support they need.

Responsible for completion of the Resident Assessment Instrument in accordance with federal and state regulations and company policy and procedures. Acts as in-house case manager by considering all aspects of the residents care and coordinating services with physicians, families, third party payers and facility staff.

Job Description and Responsibilities:

The MDS Assessor must have solid knowledge and expertise in the MDS OBRA and PDPM process including governmental and state requirements and regulations.

The MDS Assessor assumes a role to achieve optimal and accurate reimbursement. The MDS Assessor under the supervision of the MDS Coordinator shall be responsible for:

Assess residents thoroughly according to RAI manual guidelines

Must have the basic knowledge of billing, cost containment, PDPM hierarchy and associated rates, consolidated billing and denial/expedited review requirements.

Conducting concurrent MDS reviews to assure achievement of maximum allowable PDPM categories

He/she will integrate information from all disciplines to ensure appropriate reimbursement.

Maintaining and transmitting all required data concerning resident’s assessments to the appropriate regulatory agencies as required by regulation.

Upon completion of the MDS, the MDS assessor will open care plan reviews, for all initial, annual and significant change MDS’s assessments, the MDS team will open the careplans as per the triggered items for the nursing department.

Ensuring timely and effective coordination, implementation, and scheduled review of the resident’s care plans by the Interdisciplinary Team.

Ensuring timely completion of MDS prior to monthly billing and triple check for Medicare Part A and HMO’s as per policy.

Responsible for communicating clinical and reimbursement information to MDS Coordinator for Medicare and HMO information for weekly Utilization Review as per policy.

Participating in daily PDPM meetings and New Patient Review Process with all Interdisciplinary team members

Provide thorough and expert skilled need documentation reviews and communicate all discrepancies regarding documentation to the appropriate leadership team members

Educate and coach interdisciplinary team members on skilled documentation requirements

Assist with management of the Case Mix Review and OMIG Review related processes.

MDS Assessor Special Demands:

5. Assist nursing department in emergency situations as needed

6. Participate in annual competency reviews

Education and Experiences:

Must hold a current Registered Nurse License/Licensed Practical Nurse in good standing within the state of employment

Salary and Benefits: Up to $110,000 (Based on Experience)
