Overview
Working with persons with Mental Illness in a Fountain House/Clubhouse setting.
Implement individual Person Centered Plan involving skills that promote independence while instructing, modeling, assisting the consumer in daily living skills, socialization, leisure and /or recreational activities, educational and vocational skills.
Responsibilities
Required knowledge, skills, and abilities:
Knowledge of basic Fountain House / Clubhouse principles; ability to work with persons who may be different from oneself in areas of ability, age, economic status, and learning ability; empathy and understanding of consumer problems and ability to respond to those problems; ability to determine consumer needs; ability to secure cooperation of the consumer
Be familiar with HIPAA rules and regulations and abide by them.
Implement individual Person Centered Plan involving skills that promote independence while instructing, modeling, assisting the consumer in daily living skills, socialization, leisure and /or recreational activities, educational and vocational skills.
Record notes to document all services provided to the consumer on a daily basis.
Assist in transporting the consumer to community services/ activities when required, attending to the appropriate safety guidelines.
Encourage consumer to make decisions himself in as many areas as possible.
Report any changes in the consumer’s physical condition or personal needs to the PSR Director.
Notify PSR Director of any concerns of neglect, abuse, or exploitation of consumer.
Assure that consumed is never left unsupported in any area of the day program facility, in a vehicle or in any public place.
Remain focused on your consumers activities, interacting with their unit or group of members rather than with other consumers/ co-workers located in the program unless appropriate.
Operate company vehicles in a manner consistent with all applicable laws and insure vehicle is returned to the program with a quarter tank of gas and free of debris.
Inform the PSR Director of any maintenance issues.
Represent the company in a professional manner in a spirit of cooperation when in contact with director, co-workers, parents / guardians, and other persons involved in the care of the consumer.
Obtain and keep current al training required by the agency, with a minimum of 12 hours of training each year.
Understand that failure to renew training by date due will result in immediate removal from the schedule until training is brought up-to- date.
Know and maintain compliance with company policies and procedures.
Operate office equipment as necessary, i.e. calculator, copier, fax, etc.
Perform other duties as required by circumstances.
Qualifications
Must be at least 24 years old
Must have a High School Diploma or GED
Must be able to read, write, understand, and follow directions.
Must have no substantiated findings on the NC Health Care Abuse Registry
Must meet SBI and DMV requirements of LME/MCO contracts.
Must have a valid driver’s license.
Must have 3 Full years of driving experience.
Must have acceptable level of automobile liability insurance.
Experience with persons with intellectual/developmental disabilities preferred but not required.
Must be willing to complete all required trainings as assigned.
Must have access to a device with internet connection at the time services are rendered.
