Fulltime MDS Coordinator position available

Monday through Friday dayshift schedule with weekend manager on duty (MOD) rotation

To join this team please have the following requirements:

  • COVID Vaccination Required
  • Graduate of an accredited School of Nursing (RN, BSN, or LPN)
  • Current/active license for Registered Nurse
  • Minimum 3 years clinical experience in a long-term care setting
  • Prior MDS/RAI experience
  • Demonstrated clinical assessment skills
  • Knowledge of Medicare and Medicaid regulations and coverage guidelines related to skilled nursing
