Description Duties and Responsibilities –• Supports the delivery of high-quality clinical and concierge services by conducting in-home supervision of staff, including completing and documenting required employee competency checks and providing staff re-education as required.• Represents JFSA in a positive manner, conducting start of care assessments, establishing client needs and developing individualized plans of care.• Collaborates on client matters to ensure the safe delivery of individualized services, including providing routine nursing care as needed.• Ensures the development of Caring at Home personnel by working collaboratively with the Executive Director, Clinical Director, Skilled Care and agency partners to create and deliver educational initiatives; remains current with regulatory changes.• Supports quality assurance activities across the department including reviewing visit notes for accuracy and compliance with conditions of participation, agency license(s), and funders.• Prepares required documentation for accreditation and compliance audits; serves as agency contact during all audits and compliance reviews.• Submits quarterly chart audit reports to the Executive Director and the agency’s Quality Assurance/Medical Services Committee.• Addresses incidents and resolves client and employee complaints in real time, preparing follow-up reports and plans of correction as necessary.• Demonstrates an understanding of financial reporting and home care funders, including Medicare, commercial insurance, managed care programs, and other funding sources; help to ensure services are delivered as authorized.• Represents the agency professionally at all times; building relationships and rapport with internal and external entities including physicians, case managers and other referring institutions/entities.• Provides on-call clinical support, participating in an agency on-call rotation.• Acts as an educational resource for clients, families and colleagues.• Demonstrates an awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices.• Performs other duties or special projects as required or as assigned.

Salary Range $80,000 to $90,000 per year
