Job Summary: The Licensed Practical Nurse (LPN) in a Skilled Nursing Facility (SNF) plays a key role in providing comprehensive care to residents during the night shift. The LPN is responsible for ensuring the health and well-being of residents by assisting with medical needs, administering medications, and monitoring vital signs, while working closely with the healthcare team.
Duties and Responsibilities:
- Provide direct nursing care to residents, including medication administration, wound care, and monitoring vital signs.
- Assist with daily living activities and ensure that resident needs are met promptly and efficiently.
- Document and report any changes in resident health status to the Registered Nurse (RN) and healthcare team.
- Implement and follow individualized care plans in collaboration with the healthcare team.
- Maintain a safe and clean environment in accordance with facility protocols and regulatory guidelines.
- Provide emotional support and education to residents and their families.
- Ensure that all nursing practices adhere to established policies, procedures, and state regulations.
- Participate in shift handovers, ensuring accurate communication of resident care and any concerns.
- Monitor and manage supplies and medications, ensuring they are used effectively and replenished as needed.
Qualifications:
- Active LPN license in the state of New York.
- Strong clinical skills with attention to detail and the ability to respond effectively in emergency situations.
- Excellent communication and interpersonal skills, with a focus on providing compassionate care.
- Familiarity with electronic medical records (EMR) systems.
- Ability to work independently and as part of a team in a fast-paced environment.
Education: Completion of an accredited LPN program.
