Job Responsibilities:
  • Provide short-term individual counseling
  • Assess the psychosocial status of patient/caregiver related to terminal illness and communicate findings to the RN and other members of the interdisciplinary group
  • Complete social evaluations including family dynamics, caregiver abilities, communication patterns, risk for suicide, neglect or abuse, and plan intervention based on evaluation findings
  • Address patient/family questions and issues
  • Educate patient/family on and assist in preparation of advanced directives
  • Maintain a collaborative relationship with personnel to support patient care
  • Serve as a liaison between patient/caregiver and community agencies
  • Maintain clinical records for all patients
  • Active participation in team meetings
  • Demonstrate flexibility with job duties and assignments
Skills & Qualifications:
  • Master's degree from a school of social work (MSW) accredited by the council of social work education
  • Minimum of one year experience in a health care setting; home care experience preferred
  • Understand hospice philosophy, and issues of death/dying
  • Strong communication and interpersonal skills
  • Adaptive problem-solving skills
  • Compassionate and caring attitude
  • Valid driver's license, clean driving record, and reliable automobile
  • Proof of current auto insurance
  • Criminal background check clearance/fingerprint clearance
  • Current CPR certification
  • Negative TB test
