Skilled Coordinator - Job Duties:

Mon - Friday schedule 8am - 4:30pm

Occasional skilled visits with patients, various skilled care disciplines such as wound care

Working with field nurses, Director of Nursing and the office team to accept new patients and service existing ones

Participate in telephonic on call rotation on weekends and evenings

Additionally:

Perform care as designed by clients individual Plan of Care and within scope of states nurse practice act

Monitor patients condition and notify appropriate personnel of any changes

Providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes

Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process

Prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous and intramuscular medications. Observe, report, and record adverse reactions to medications and/or treatment to the registered nurse

Perform wound care and recognize the need for and institute emergency measures in a rapid and appropriate manner

Follow principles of infection control and Universal Precautions

Treat clients, family, and staff with courtesy and respect

Requirements:

Current unrestricted LVN license in state of Texas

1+ years of experience on your license required

1+ years' Home Care experience preferred

Reliable transportation / valid drivers license / auto insurance

Current CPR/ BLS

Negative TB test
