Praesum Health is seeking a Veteran Engagement Specialist (VES) to focus his/her efforts on the veteran population who face addiction challenges. In this role, you will help develop and implement strategies to identify sources of patient referrals within the veteran community across the Praesum Health system and facilitate the ability of these individuals to engage care within one of the Praesum Health’s service offerings. This is an inside customer coordinator position within a defined territory to initiate contact, develop and maintain relationships with both veteran’s service organizations and the veterans they serve, and ensuring these individuals are aware of their care options as prescribed by the VA. You will assist in the development and execution of strategies and plans aimed at increasing awareness of, and referrals to Praesum Health’s comprehensive addiction inpatient and outpatient facilities. In the Praesum Health system, these clinics are all credentialed to serve veterans as part of the VA’ s Community Care Network.

As a member of our Client Engagement team, you will contribute to team targets and goals focused on Praesum Health facilities and growing veteran census. At all times, the VES will promote Praesum Health services and reputation in a professional and ethical manner.

As the VA authorizes care and refers veterans to Praesum Health, this authorized care is completely covered by the VA under the CCN program. The VES works with the VAMC’s and VA caregivers in support of their efforts to match veterans with the care they require. Praesum Health VES will assist veterans to identify and engage VA healthcare resources. In addition, you will ensure veterans and are provided exceptional support, education, and customer service from Praesum Health.

Veteran Engagement Specialist Duties/Responsibilities: • Assist veterans to learn more about their care options through the VA • Facilitate patient education around VAMC services and the VA’s Community Care Network care access • Serving the needs of the veteran community, the VES will be responsible for identifying and developing partnerships with veteran organizations, state and local government entities, and other organizations to strengthen relationships that will support the mission of the VAMC’s, as well as their Community Care Network (CCN) to better serve veterans • Thorough understanding and firm knowledge of Praesum Health’s treatment settings and ability to use the knowledge and understanding to build relationships and increase patient referrals. • Create and sustain positive impressions and relationships with referral professionals, optimizing Praesum Health messages and methods • Plan and organize daily activities to optimize the use of time and maximize the achievement of organization’s strategic goals for the veteran segment. • Work collaboratively with team and other departments to meet all the organization’s strategic goals. • Assist in design and implementation of successful marketing campaigns for the veteran treatment segment. • Ability to record and analyze progress, areas of opportunity, and needs with all stakeholders (TPA and the VAMC’s).

Veteran Engagement Specialist Requirements: • BS/BA degree required with a minimum of 2 years healthcare marketing or related experience. • Computer proficiency, social media knowledge and ability to utilize all forms of technology is required. • Veteran of the U.S. military preferred or extensive experience as a first responder • Must be motivated and able to work with minimal supervision. • Strong verbal and written communication skills including but not limited to accurate and timely reporting, documentation of activities, response to emails, phone calls or other messages.
