The practice, known as Catholic Health Physician Partners Internal Medicine Associates
Doctors in the practice are Maria Brountzas, MD; Jane Serio, MD; Florence Barricelli, MD; Doris Berland, MD; and Judah Guy, MD. Working together for many years, the physicians bring decades of experience to new neighborhoods while remaining dedicated to the patients who depend upon them.
Welcoming these five skilled, experienced physicians to CHS furthers our commitment to providing quality medical care to Long Islanders and is integral to our strategy in population health management, said Catholic Health Services Medical Group Chief Administrative Officer Sunny Chiu. The new office is on the front lines of CHSs larger network, which includes physician offices, diagnostic testing centers and some of Long Islands most distinguished hospitals and skilled nursing facilities.
The CHS Physician Partners Medical Group is CHs employed faculty, comprising approximately 450 physicians and mid-level providers in more than 90 community-based locations, spanning from Manhattan to Long Islands East End. The medical group includes physicians practicing a broad range of medical, surgical and pediatric specialties, with many recognized as regional and national experts in their fields.

POSITION SUMMARY: The Advanced Practice Provider, APP (Physician Assistant/ Nurse Practitioner) is an integral part of the delivery of health care and provides for primary care in many ambulatory settings through the Catholic Health Physician Enterprise. The APP provides patient care under the supervision of a qualified physician and is credentialed through the Medical Staff process.
Duties include, but are not limited to: obtaining a detailed medical history, performing a comprehensive physical examination, diagnosing medical problems, ordering and interpreting diagnostic studies, instituting appropriate medical treatment, ordering medications, performing invasive procedures, education and counseling of patients as credentialed. Work load: 60% administrative, 40% direct patient care.
POSITION REQUIREMENTS AND QUALIFICATIONS:
Requirements:
- PA - Completion of an accredited Physician Assistant program, minimum Bachelors degree required
- NP Current RN license and current AANP or ANCC certification
Certification:
- Physician Assistant Certified (PA-C) or Nurse Practitioner certification (ACNP)
- BLS and ACLS
PERFORMANCE RESPONSIBILITIES
- Collaborates with and works under the supervision of the physician in the provision of patient care.
- Follows established policies for treatments and procedures.
- Greets and welcomes patients. Uses two (2) identifiers for patient identification (name and date of birth). Completes detailed history and physical examination on patients. Assesses patients for basic vital signs, chief complaints and purpose of visit and reviews charts for recent visits, chronic problems, recent and outstanding laboratory studies, and other information that would facilitate prompt and complete care.
- Provides telephone triage for patients requesting information or advice and assisting with and encouraging appointments when appropriate.
- Obtains additional data from patient/significant other, EKG/lab/ diagnostic test results and other historical data.
- Evaluates patient responses in relation to established goals and contributes to the revision of the plan of care.
- Writes appropriate orders, prescribes/renews prescriptions and provides patient education.
- Reviews laboratory findings and diagnostic testing for follow up, addressing critical findings in a timely manner.
- Assists with creating provider and practice level quality performance reports.
- Coordinates a wide range of self-management support and education to foster patient engagement and activation.
- Delivers interventions based on standard orders and established written protocols to improve the health of the patient population.
- Works collaboratively with members of the healthcare team to develop tools and interventions designed to achieve optimal patient outcomes.
- Prepares, administers and documents medications and vaccines.
- Collects, prepares and processes laboratory specimens for on-site and remote-site testing. Tracks and enters results in medical record. Performs quality control for waived testing and log maintenance.
- Performs, prepares for and assists with in-office procedures and diagnostic testing.
- Assists with the scheduling of procedures, referrals and specialty services and testing as needed.
- Maintains and uses principles of aseptic techniques and infection control when performing clinical duties and tasks.
- Demonstrates ability to critique own practice and accepts constructive feedback regarding performance. Pursues corrective action.
- Participates in team meetings/huddles to foster communication and provide feedback to providers, patients, families, pharmacies, vendors, etc.
- Aids in the orientation of new staff. May precept student clinicians.
- Works with healthcare team to assure smooth and efficient patient flow and clinical operation.
- Works to create and maintain excellence in patient/family satisfaction, relationships and engagement. Responds to care questions or complaints for timely resolve. Provides documentation on unresolved issues voiced by patients and refers to appropriate party for resolution.
- Operates clinical equipment according to manufacturers instructions including safety and quality control checks. Checks care areas for malfunctioning equipment and general maintenance problems on a regular basis. Reports findings to appropriate person or arranges for repairs, as required.
- Ensures that blood/body fluids, biohazardous materials, needle containers and potentially infectious waste are appropriately disposed and removed from the clinical area according to infection control guidelines.
Posted Salary Range:
USD $66.55 - USD $90.00 /Hr.
