Currently, we are recruiting experienced Staff Pharmacists to join the Polaris team.OVERVIEWThe Pharmacist is responsible for the day-to-day operational duties associated with the dispensing of prescription medication as required to meet the needs of a long-term/post-acute care pharmacy. The Pharmacist must adhere to the company's quality and customer service standards, policies, and procedures, as well as the State and Federal guidelines.RESPONSIBILITIESVerification for completeness and accuracy of all new and refill labels and orders. Check for content and accuracy of all medication packaging and labeling including emergency drug kits and IV medicationsMaintain resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including OTC and home remedies)Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances; assist in maintaining a perpetual inventory for schedule II drugsWork with appropriate pharmacy technician in reconciling Controlled Substances perpetual inventory as required by management for compliance with state and federal regulationsProvide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customersDocument all communication with physicians, nurses, other members of the healthcare team, and patients/residents and their family members in the medical recordPreparation and/or supervision of all sterile IV drug products or solutionsProvide evening, weekend, and holiday coverage during rotation and as required by managementFollow all applicable government regulations including HIPAAOther duties as assigned; Job duties may vary depending on business needsResponsible for the flow of work and appropriate outcomesQualificationsQUALIFICATIONSCurrent Pharmacist license active and in good standing in the state of practiceManagement experience preferredBachelors degree or Pharmacy Doctorate from an accredited School of PharmacyPerform accurately and efficiently when dispensing prescription medicationsAbility to work flexible hours, which may include evening, weekends, and holidaysExperience using FrameworkLTCExperience using Microsoft Word, Excel, and Microsoft OutlookMinimum of two (2) years recent experience in an acute-care setting or one (1) year experience as a production pharmacist preferred (not required). Experience and knowledge base of intravenous admixtures services and intravenous therapy desired.Excellent interpersonal skills and well-developed verbal and written communication skills with English as the primary language.Ability to work independently as well as working in a team environment, meets deadlines, establish priorities, and be flexible, work under pressure with multiple tasks.
