Description

Position Summary:

Under general supervision, the Care Coordinator operates as part of the Care Team, working collaboratively with providers and clinical staff, internal and external, to support patients with chronic conditions and complex needs. They may serve as a liaison for needed social services, and are responsible for coordinating referrals, consults and diagnostic testing as well as tracking results of these appointments. Under general supervision they may help with prescription medication assistance applications as well as other benefits. They are also responsible for communicating with clients that are considered overdue for care.

Job Responsibilities:
  • Demonstrates and applies knowledge of the philosophy/principles of culturally sensitive and comprehensive coordination of care. Maintain confidentiality and adhere to HIPAA regulations in handling client health information.
  • Maintains active contacts with community partners to ensure easy facilitation of referrals.
  • Maintains regular contact with other social service agencies and health care providers involved with client care to provide information and obtain feedback on client's overall progress.
  • Responsible for engaging with external community partners regarding client care (e.g. scheduling /re-scheduling appointments, follow-ups, monitor no-shows) to assist with service connection to improve physical & behavioral health outcomes.
  • Coordinate referrals from internal and external sources, ensuring timely and appropriate linkage to care. Initiate the referral process and track the progress of referrals to ensure seamless service delivery.
  • Establish and maintain relationships with external partners and community organizations to expand our network of resources.
  • Effectively communicates with health care team within the clinic and the community to provide quality services.
  • Collaborate with healthcare providers, social workers, and community resources to facilitate access to services and continuity of care.
  • Contact clients overdue for care and encourages appropriate follow up care. Provide support and guidance to clients and their families on available resources and services.
  • Documents all contacts and follow ups with clients and other providers daily. Maintain accurate records of all referrals and services provided, ensuring confidentiality and compliance with regulations.
  • Monitor and review Electronic Health Records (ECR) of local hospitals to identify emergency department visits. Ensure compliance with HIE protocols and regulations set forth by HIPAA and other relevant guidelines.
  • Take and record patient vital signs such as blood pressure, temperature, pulse rate, and oxygen saturation. Prepare patients for examination and treatment by healthcare providers.
  • Assist with routine laboratory procedures including specimen collection, labeling, and processing.
  • Performs miscellaneous duties as assigned.
Qualifications

Minimum Qualifications:
  • High school graduate with at least 3 years of experience in a generalized public health care setting.
  • Attention to detail and commitment to client safety and confidentiality.
  • Proficiency in computer skills, including electronic health record (EHR) systems.
  • Ability to work independently and collaboratively within a team environment, demonstrating flexibility and adaptability.
Preferred Qualifications:
  • Associate's degree with 1 year of health care experience or completion of a medical assistant program preferred.
  • Strong interpersonal and communication skills.
  • Skills in using word processing software for composing letters, memos, etc.
  • Bilingual, Spanish speaking preferred
