**Wellness Nurse Job Description** Provide high-quality care to residents in a senior living community, promoting their overall well-being and quality of life. **Job Responsibilities** * Identify ongoing needs and services of residents to deliver the highest quality resident services * Ensure proper follow-through, assessment, and documentation for residents with changes in healthcare services and status * Conduct monthly wellness visits for all residents and assess, monitor, and follow up with residents who have experienced falls or incidents * Maintain communication with residents' families and responsible parties to ensure medical needs are met * Ensure weights and vital signs are obtained monthly for each resident and maintain medical supplies and emergency kits * Provide clinical support and assistance to community team members as needed * Understand and follow infection control practices and promote infection control standards * Regularly monitor each resident's medication and treatment profile to ensure accurate administration **Qualifications** * Graduate of an approved college or school of nursing * Current state license as a professional Registered Nurse (RN) * Minimum of one year experience in home health, assisted living, hospital, or long-term care environment * Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care * Ability to handle multiple priorities and possess written and verbal skills for effective communication * Competent in organizational, time management skills, and good judgment, problem-solving, and decision-making skills **About Sunrise Senior Living** Sunrise Senior Living has championed quality of life in senior care for over 30 years.
