Amergis Staffing is hiring for a Care Coordinator who has experience assisting members with disabilities. They will work in collaboration with the multi-disciplinary team. They will have input in the development of the plan of care for those patients requiring social work intervention. Provides medically necessary intervention to patients who have social or emotional problems which are or are expected to be an impediment to the effective treatment of the patient's medical condition and/or rate of recovery.

+ Location: Chicago, IL

+ Weekly pay + benefits offered

+ Work Schedule: Monday-Friday only (8am-430pm)

+ License & vehicle required for case visits

Responsibilities:

+ Visit members in the home to complete an assessment, develop a care/service plan that meets members needs and provide periodic follow-up

+ Verifies eligibility for wavier services

+ Documents findings in the Care Management System

+ Meets or exceeds timelines associated with case activities

+ Provides succinct presentation of member, and solidifies plan of care with interdisciplinary Care Team

+ Completes a member assessment according to department policy and procedure.

Minimum Requirements:

+ Must be Licensed in the State of Illinois in one (1) of the following qualifications: Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC), Licensed Professional Counselor (LPC), OR Bachelors degree or higher in Human Services related field from an accredited college or university

+ Two (2) years of recent experience providing services and support to community based persons with disabilities

+ Duties may involve travel to locations throughout the Cook County area

+ COVID-19 Vaccine required, no exemptions
