Key Responsibilities:
  • Provide community-based mental health services to clients through assessment, diagnosis, treatment planning, and intervention implementation.
  • Collaborate with clients, families, and stakeholders to develop and implement personalized treatment plans tailored to specific needs and goals.
  • Utilize evidence-based practices and strengths-based approaches to empower clients and build resilience.
  • Coordinate care with other service providers, including primary care physicians, psychiatrists, schools, and community organizations.
  • Maintain accurate and timely records of client interactions, treatments, and progress.
  • Participate in staff development activities to enhance knowledge and skills related to youth mental health services.
  • Contribute to the organization's quality improvement initiatives through data collection, analysis, and reporting.
  • Foster collaborative relationships with team members, stakeholders, and professionals involved in the client's care.
  • Facilitate PSR group sessions aligned with client treatment plan goals.

Additional Qualifications:
  • Must be at least twenty-one (21) years of age to operate a company-owned vehicle.
  • Must be at least twenty-three (23) years of age to operate a company-owned fifteen (15) passenger van.
  • Knowledge of case management, rehabilitation methods, and principles related to mental health and/or substance use disorders.
  • Ability to problem-solve, communicate effectively, and produce clear and concise reports.
  • Ability to multi-task, coordinate services from community resources, and maintain effective communication with staff.
  • Ability to establish effective relationships with clients and their families.
  • Ability to follow directions and accept supervision.
  • Valid driver's license in state of residency with an acceptable driving record.
  • Reliable means of transportation and proof of valid and current auto insurance.
