Description

The Admissions Director is responsible for managing all aspects of the patient intake process and admission process including managing the members of the intake team (clinical and non-clinical), establishing, and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

Supervisory Responsibility

Responsible for site-level clinical team members. Depending on site size, teams/positions responsible for may include Intake, CNA, Nursing, Support Service, and/or Therapy.

Responsibilities

  • Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
  • Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
  • Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
  • Establishes and maintains positive working relationships with current and potential referral sources and the hospice marketing team.
  • Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for care, plan of care initiation, and coordination of care with multiple service providers.
  • State Admissions Director for both Brookfield and Madison locations
  • Ensures maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
  • Builds and monitors community and customer perceptions of the Hospice as a high-quality provider of services.
  • Gathers, collates, and reports referral statistics including key customer referral trends.
  • Oversees and manages Key Performance Indicators tied to a successful admission process, including but not limited to: days to admit, conversion percentage, non-billable days, and CAHPs data reflecting admission education.
  • Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
  • Maintains comprehensive working knowledge of the company's contractual relationships and ensures that patients are admitted according to contract provisions.
  • Participates in quality assessment performance improvement teams and activities.
  • Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by the company.
  • Oversee and manage Intake Coordinators and day to day workflow.
  • Oversee and manage the admission nurses schedule and workflow day to day.
  • Oversee the admission process, including initial plans of care, admission consents, and the admission audit process.
  • Perform additional, related duties as needed.
  • Budget Authority and Responsibility: No authority over any portion of the budget.
Qualifications
  • Education: Bachelor's degree in nursing, Master's Degree in health care or related field preferred
  • Experience: At least three (3) to five (5) years' experience in health care management, preferably in hospice or home care.
  • Licensing/Certifications: Registered nurse with a license in the state(s) practicing.
