The Long-Term Acute Care Registered Nurse provides planningand delivery of direct and indirect patient care to medically complex patientsin long-term acute settings.
Essential Duties and Responsibilities:
+ Provides planning and delivery of direct and indirectpatient care
+ Responsible for providing care to medically complex patients
+ Supervises care, treatment, and services of the patient(s)
+ Documents the patients plan of care using identifiednursing diagnosis, expected patient outcomes and selected nursing interventions
+ Maintains confidentiality at all times with patients,medical staff and co-workers
+ Provides a safe, comfortable and therapeutic patientenvironment
+ Adheres to and supports thepolicies/procedures/goals/objectives of the hospital in an attempt to providequality patient care
+ Revises plan of care according to evaluation, changes inplan of care and nursing interventions
+ Performs interventions according to identified priorities,plan of care, and facility policies and patient care outcome standards
+ Identifies and prioritizes actions based upon patient carerequirements
Minimum Requirements:
+ Current RN license for the state in which the nursepractices in
+ One (1) year nursing experience in a long-term acute carefacility preferred
+ Experience in acute rehabilitation for vents, wound care,cardiac, and stable post trauma patients
+ Complies with all relevant professional standards ofpractice
+ Participation and completion of Amergis' Competency programwhen applicable
+ Current CPR if applicable
+ TB questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or stateregulation)
+ Must meet all federal, state and local requirements
+ Successful completion of new hire training as applicable tojob site
+ Understand patient confidentiality and HIPAArequirements
+ Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
+ Computer proficiency required
+ Must be at least 18 years of age
