Case Manager

An established organization that works to provide excellent social services to the geriatric population is currently seeking qualified Case Managers to join their interdisciplinary team of professionals in Brooklyn. You will be surrounded by great leadership and a team of interdisciplinary professionals who will be there to train and guide you in your professional journey.
The base pay range for this position is $22 to $24 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. This is a temp-to-perm role; you can gain access to company benefits, paid time off, and more
Responsibilities:
* Initial screening of applicants during case intake
* Program eligibility assessment for applicants
* Emergency intervention triage
* Identification of short-term needs
* Securing other social services, including government benefits, as needed
* Developing and implementing a comprehensive written case plan
* Providing a full range of social services for older adult clients
Qualifications:
* Bachelors Degree in social work or related field
* Prior experience as a case manager
