Job DescriptionThe Nursing Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately according to state and federal guidelines. Ensure that patient and family care services are delivered appropriately and for the supervision of home care medical and support staff. They will be a resource person by providing subject matter expertise and quality practices for home health services and provide education and training, as applicable. They will be expected to review and monitor operations and clinician activity reports and address areas for improvement for a plan of action.Responsibilities:Promotes the mission, vision, and values of the organizationProvides guidance and assistance to staff in all matters relating to the delivery of home Health/hospice services to assure that regulatory, quality, and practice standards are maintained. Is responsible for the day to day operations and assist with scheduling of all field clinicians under their supervision.Assist clinicians with maintaining a safe, patient-family centered careAvailable at all times during operating hours or ensures alternate backup is availableDevelops and cultivates collegial working relationships with physicians, leadership team members, and community partnersPrepares and leads case conference/IDT meeting with Homecare/Hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.Reviews and manages case referrals by ensuring they are appropriate to the service line and meet eligibility requirements Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, in-services, goals and objectives.Assist with identification, measuring, analyzing audit process to ensure continuous improvement safeness in clinical care. Communicate outcome data and provide education/training to organization personnel as applicable.Evaluates performance of team members under their direction; coaches, counsels and disciplines within accordance of Human Resources policiesAssists in the screening and interviewing process of new employeesOnboard and mentors new staff, as applicableParticipates in cross-covering duties for alternate Homecare/Hospice Nursing Supervisor as neededParticipates in clinical and/or administrative on-call as required.Participates in leadership and committee meetings as assignedReports to and assist the director in overall management of the department.Apply DRH Standards of Behavior and exhibit behaviors consistent with DRH core values.Performs other related duties as assigned.Shift: 1Hours: 8a to 5p, M-F, 80 hrs pp, plus callType: Full TimeCategory: ClinicalRequirementsMinimum Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. A minimum of two (2) or more year's management or supervisory experience in that setting is preferred. Familiar with local, state, and federal Homecare and/or Hospice general process, Conditions of Participation and claims processing requirementsStrong professional work ethic and team player with effective critical thinking skills with ability to work within an interdisciplinary setting.Demonstrates excellent observation, good nursing judgment and communication skills. Ability to read, analyzes, and interprets common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to medical providers, leadership team, and public groups, Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course to determine appropriateness of admission and assist with an effective plan of careFamiliar with QAPI process; experience preferredCertifications, Licenses, Registrations: Current OK/Multi-State Registered Nurse license, Basic Life Support certificate, current valid driver's license and automobile liability insurance must be maintained.*Safety-Sensitive Position -This is a "Safety-Sensitive" Position as defined by Oklahoma law.
