Its service area also reaches into southern Virginia. With more than 600 team members, the hospital offers a broad range of medical and surgical services. It features a 24-hour emergency department and offers advanced diagnostics as well as specialty and primary care.Position Summary The Nurse Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family.The Nurse Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care.
