SUMMARY

The Clinical Manager of Social Work, under the guidance and support of the Director of Care Coordination, will lead, support, and direct the social work team and their clinical processes, and support efforts to integrate the team and their functions seamlessly into the hospitals system of care. The Clinical Manager will focus on upholding core competencies and streamline workflows,with an overarching goal of improving the patient experience.The Manager will promote behavioral health, wellness and assessment throughout the institution and it's programming with the support of teams leads at the Center for Care Coordination. The Clinical Manager of Social Work will seek to identify systems building opportunities that will promote optimal child health using data to drive program development. The Clinical Manager works as part of the management team with the Director to move the care coordination model forward and establish best clinical practice workflows using a strength based framework.

Responsibilities

ROLE RESPONSIBILITIES

Direct oversight and management of the inpatient, outpatient, community, and behavioral health social work teams to ensure the highest standards of consistent clinical care, including the establishment of baseline interventions and documentation. Develop team skills including effective communication and problem-solving in an effort to cultivate staff leaders, prioritize goals and promote internal communication.

Effectively enhance the care coordination model through strength-based programming and consistent quality clinical measurements (including chart reviews, surveys, interviews) all which assure high caliber practice. Identify and eliminate barriers and obstacles and/or policies and procedures that inhibit patient satisfaction.

The manager will have proven experience in program development, cross-sector project leadership and a strong understanding of the Care Coordination Model. The role will be responsible for driving and supporting clinical outcomes using data and the ability to provide rapid response analysis of a real-time system, while seeking knowledge from team leaders and management.

Work collaboratively with CCSG leadership and organizational management to implement and assess specific needs of unique clinic settings and their psycho-social work/behavioral health needs.

Work collaboratively with ED leadership to manage the specific needs of the Behavioral Health population. Participate in rounding, and other team meetings for ongoing awareness, and management of complex cases in the ED.
Participate in building out the role for clinical care coordination as part of the CIN project- particularly the need for social work presence or consultation. Responsible for exploiting opportunities and employing various strategies of support and response at the macro and community level.

Represent the organization and its mission while working collaboratively with community partners to develop relationships that improve service delivery, and increases access and support for optimal patient care. Provide feedback and partnership experience to community stakeholders through project development, training, and relationship building.

Experienced behavioral health background to allow for clinical review of programs, projects, assessments, research and best practice integration. A strong understanding of behavioral health applications, and the role it plays in patient care and optimal child health.

Work closely with team leads at the Center for Care Coordination, as well as within the hospital system to move the behavioral health model to a more therapeutic response of patient care, while addressing and supporting team through demanding caseloads and logistical challenges. Partner with behavioral health teams to create solutions and safeguards towards a healthier population and an institutional goal of Zero Suicide.

Development of on-call protocols, expectations, and process. Provide on-call support to social work staff, (10% of effort) and engage Director in decision making as appropriate. / Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the CT Children's

SUPERVISORY RESPONSIBILITIES

Provides direct supervision to assigned team leads. Provides supervision and guidance as needed to inpatient, outpatient and behavioral health social workers in all operations and their overall performance standards. Guide and mentor team members, promoting their professional development, and contributing to performance appraisals when it is applicable.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

KNOWLEDGE OF: Experience in foundations of social work and best practice implementation in a variety of settings focused on improved child and family health and wellness and the application of the care coordination model throughout the institution.

SKILLS: Exhibits leadership skills to promote and move social work forward as a discipline and as a integrated system of support. Ability to work at a high level of strategic management over complex patient care, the care coordination model, and a community of support. Expertise in a systems perspective of how clinical care coordination can enhance wellness and improve health outcomes as well as influence micro and macro change. Experience as a strong team influencer.

ABILITY TO: Participate in and lead a diverse group of providers in an effort to merge systems and programming for optimal patient care and to support and promote the hospitals vision and mission. Demonstration of leadership skills including excellent written, oral, and presentation skills.

Qualifications

EDUCATION and/or EXPERIENCE REQUIRED

Education Required:

Master Degree in Clinical Social Work

Experience Required:

A minimum of five years of clinical supervision as well as program development and leadership experience in clinical environment. Demonstrated experience developing and implementing projects that lead to an enhanced patient experience. Excellent oral and written skills. Proven ability to think strategically, utilize a systems lens when making decisions, display sound judgement and problem solving skills particularly in high acuity situations, and work across teams and disciplines. Behavioral health background to support clinical integration and crisis response. Track record of strong organizational and management skills.

LICENSE and/or CERTIFICATION REQUIRED

Current CT state social work clinical license (LCSW)
