Float Pool team members will support the facilities in the Winston Salem Market including Forsyth Medical Center, Thomasville Medical Center, Clemmons Medical Center, Kernersville Medical Center and Medical Park Hospital. With world-class technology, personal connections, and care that's easier to access and understand, Novant Health is with patients every step of the way on their journey to health and wellness.Essential functions include:Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems, Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care Makes appropriate evidence-based, patient- centered medication recommendations Participates in the management of medical emergencies Provides discharge medication review, reconciliation, and counseling as appropriate Maintains proficiency in and actively engages in central dispensing functions and operational programs Oversees pharmacy operations and technician practice/activitiesMaintains proficiency in preceptor roles and actively emulates education/ mentorship skillsWhy Novant Health?Come join a remarkable team where quality care meets quality service, in every dimension, every time. QualificationsEducation:4 Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required. Additional skills required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed.
