Implement infection control practices, safety measures, and emergency protocols to protect patients, caregivers, and oneself from harm, adhering to established guidelines and protocols. Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to review patient progress, identify barriers to care, and enhance care coordination and outcomes. Qualifications: Graduate of an accredited practical/vocational nursing program with a current license to practice as an LPN or LVN in the state of employment. Minimum of 2-3 years of nursing experience, preferably in home health, community health, or a related healthcare setting, demonstrating proficiency in skilled nursing procedures and clinical assessments. Strong clinical assessment skills, critical thinking abilities, and proficiency in administering medications, treatments, and procedures safely and effectively. Excellent communication, interpersonal, and patient education skills, with the ability to establish rapport with patients and families from diverse backgrounds and cultures. Knowledge of nursing principles, practices, and standards of care relevant to home health nursing, including regulatory requirements and best practices in community-based care. Ability to work autonomously, manage time effectively, and prioritize tasks in a dynamic and fast-paced home health environment. Proficiency in using electronic health record (EHR) systems and other technology platforms for documentation, communication, and care coordination. Valid driver's license, reliable transportation, and willingness to travel to patients' homes within the designated service area. CPR certification and other relevant certifications may be required, depending on state regulations and organizational policies.
