Job Title: Patient Care AdvocateJob Description
As a Patient Care Advocate, you will work closely with members and providers to address care gaps and remove barriers to care, enhancing both member and provider experiences. This role involves outreach and face-to-face interactions at large IPA and/or group practices. You will collaborate with providers to improve HEDIS measures and provide crucial education about HEDIS measures and coding. Your role is pivotal in scheduling doctor appointments, connecting members with community resources, and maintaining confidentiality of all sensitive information.
Hard Skills
+ 3+ years of nursing experience (RN, LPN, HEDIS, etc.)
+ Valid NJ license
+ Experience with medical terminology
+ Ability to conduct telephonic outreach and health coaching
Soft Skills
+ Excellent communication skills
+ Strong organizational skills
+ Ability to educate and advocate for members
+ Capacity to work independently and collaboratively
Job Type
This is a Contract-to-Hire position with a duration of 30 Weekly.
Work Site
This is a hybrid position requiring some days on-site in New Jersey and some days remote.
Work Environment
This is a contract position starting on 8/13/24 with potential for hire, ending on 2/28/25. The role requires working 40 hours per week from 9 AM to 5 PM, Monday to Friday, with a flexible start time of 8:30 AM if needed. The position is embedded in provider offices to schedule member appointments or conducted from a home office for member calls.
