SUMMARY The Speech Language Pathologist is an autonomous licensed health care professional who provides screening, identification, assessment, diagnoses, treatment, intervention and follow up services for disorders of speech, voice, language, communication and swallowing disorders. The Speech Language Pathologist is responsible for evaluating patients referred for service, establishing a plan of care, providing Speech Language therapy services based on the plan of care to obtain short and long term functional treatment outcomes. Speech Language Pathology services are provided to outpatients, skilled nursing residents, medical surgical patients and contracted home healthcare referrals at Mountains Community Hospital. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION/EXPERIENCE Must be a graduate of a school approved by the American Speech and Hearing Association. Must have at least a Master's Degree in Speech Language Pathology. Preferred one to two years experience in outpatient, skilled nursing and/or home healthcare settings. CERTIFICATES, LICENSES, REGISTRATIONS Current Speech Language Pathology license issued by the State of California. Current CPR certificate. Current California Drivers License. (Home Care contracts) Current Car Insurance. (Home Care contracts)
