Riverside Care of Florida is seeking a detail-oriented and organized Care Coordinator to support our home health office in Brandon, FL. Under the supervision of the Manager/DON, the Care Coordinator is responsible for accurate and efficient staff assignments, scheduling and client relationship management, monitoring, delegating and supervising care communication and resolving scheduling challenges.The Patient scheduling coordinator/ Orders management Specialist is also responsible for processing care orders accurately and timely, maintaining accurate records and providing administrative support to ensure efficient order management. This position requires strong communication skills, attention to detail, and the ability to multitask in a fast-paced environment. The ideal candidate must be able to speak English and Spanish.ResponsibilitiesUse a web-based scheduling application for all schedules.Ensuring sufficient Clinical staff availability at all times.Coordinate with the clinical team to ensure timely initiation of patient care servicesCoordinates care with caregivers, family members, and other care providers.Maintain a current patient roster, with necessary information.Prepare weekly schedule for field staff and provide copies for distribution.Control and monitor schedule changes.Obtain weekly visit count and report results to the Clinical Manager.Check compliance of visits done (on master schedule), after all notes are matched to charge; bring any scheduling problems to immediate attention.Assist in relaying messages to field staff, office staff, and community liaisons.Schedule caregivers for hours of service on new clients and open hours/shifts on existing cases. Provide consistency in caregiver scheduling with clients.Fill open shifts and secure coverage for call-offs or changes in clients' needs.Maintain ongoing communication with Managers, Caregivers & Clients regarding scheduling and changes.Communicate with physicians, nurses, and other healthcare professionals to obtain necessary documentation and clarify orders as neededCommunicate with customers regarding order status and discrepanciesAssist with HR functions such as hiring/recruiting and orientation.Maintain accurate records of all orders received and processedCoordinate with the clinical team to ensure timely initiation of patient care servicesFollow up on outstanding orders and escalate issues as necessary to ensure prompt resolutionPerform general administrative tasks as needed to support the operations and order processing workflow of then office.Qualifications:High school diploma or equivalentAloraPlus knowledge is a plus.Computer literacy with proficiency in data entry and order processing systemsPrevious experience in an administrative role, preferably in Home Health or similar setting a plus.Excellent communication skills, in English & Spanish, both written and verbal.Ability to type accurately and transcribe information effectivelyStrong organizational skills with attention to detailWillingness to learn and adapt to new processes and technologiesProficiency in Microsoft Office Suite (Word, Excel, Outlook)Excellent telephone and communication skills are necessary.
