CCFHS is looking for an LPN to join our Medical Practice working in our Colebrook, Berlin and Gorham locations This position is between 36 and 40 hours per week. Full-time Benefits Available. Generous Earned Time benefits accruing up to 4 weeks per year SUMMARY The LPN shall assist the medical provider in the provision of comprehensive primary care services. Responsible for managing, tracking, collaborating and coordinating patient health care needs including medical, behavioral health, oral health, emotional health, etc. with an emphasis on the whole patient. Some essential job duties as follows: Organize and complete documentation. Keep the flow of the clinic structured and on schedule. Counsel clients on anatomy and physiology, health maintenance and other health care issues. Maintain and record applicable client information in electronic medical. Assist medical provider with procedures and serve as a chaperone. Maintain clinical tracking for client services. Actively participate in the development, implementation and evaluation of each client's plan of care. Set up exam rooms. Clean laboratory/exam rooms and instruments after client visit. Replenish supplies and request medical supplies to the purchaser as needed. Dispense non-prescriptive medication/contraceptives per pharmacy guidelines. Provides triage of urgent/same day or next day appointment needs following triage guidelines, protocols and standing or Facilitates med refills, including obtaining prior authorizations Oversees medication samples available for patients Oversees vaccines provided to patients of the practice Provides on-site clinical support of practice nursing staff and assists in staff evaluations Works closely with CCFHS's Nurse Manager to understand and follow organization policies and procedures and Assists the local Operations Manager by providing input and oversight of specific clinical processes Qualifications Must be eligible to work in the U.S. Must be willing to submit to a background check. Valid NH LPN license required. Must have reliable transportation and valid driver's license for travel between clinic locations. Prior experience in primary care preferred. Experience with Athena Electronic Health Record a plus.
