Job Description

Description of Primary Responsibilities
  1. Responsible for supporting the health care team through community outreach, enrollment assistance, and education activities, as assigned.
    • Provides information on health insurance, safety programs, and community resources.
    • Conducts intake interviews to determine eligibility for Medi-Cal, Covered California, and Sliding Fee programs, as assigned.
    • Attends relevant community meetings as assigned.
    • Develops recommendations for improvements to the community health care system.
  2. Responsible for assisting supervisor with reporting program/grant activities, as assigned.
    • Collects, tracks, and provides reports on participation in outreach, enrollment, and grant activities as assigned.
    • Conducts community focus groups and surveys for the Network.
    • Provides various reports on community updates and community feedback.
  3. Organizes and participates in network hosted events, community events, and presentations.
    • Provides training on health related topics as assigned.
    • Provides health screenings at events.
  4. Responsible, as assigned, to support supervisors in the implementation of organizational program and grant funded initiatives.
    • Supports the implementation of program curriculum and/or objectives in community settings.
    • Supports supervisor in the collection of data and reporting.
  5. Performs other duties as assigned.
Description of Primary Attributes

Professional & Technical Knowledge:
  1. Possesses work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, technical or health care related knowledge frequently acquired through completion of a trade school, para-professional, or certificate type program.
  2. Experience in community outreach or educational activities preferred.
Technical Skills:
  1. Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs and other elements.
  2. Ability to use Microsoft Excel to review and compile data, including the use of formulas, functions, lookup tables and other standard spreadsheet elements.
  3. Ability to create basic presentations in Microsoft PowerPoint.
Licenses & Certifications:
  1. Valid CA driver's license.
Other Requirement: As a condition of employment at Family HealthCare Network, all employees assigned to provide Covered California education and enrollments must obtain certification as a Certified Enrollment Counselor, which will include successfully completing State mandated training, passing state certification exams, and passing a Federal Background Check.

Communications Skills:
  1. Job duties require the compilation of information prepared in effective written form, including correspondence, reports, articles or other documentation.
  2. Effectively conveys technical information to non-technical audiences.

Pay Scale:

Min Hourly Rate: $21.56

Max Hourly Rate: $30.86
