We are seeking a highly skilled Clinical Pharmacist to join our Critical Care and Complex Care Service team at Mass General Brigham.

The successful candidate will be responsible for providing expert clinical pharmacy services to patients in critical care and complex care settings, including the optimization of drug therapy, medication reconciliation, and patient education.

• Provide direct patient care and clinical pharmacy services to patients in critical care and complex care settings
• Optimize drug therapy and medication regimens to improve patient outcomes
• Conduct medication reconciliation and ensure accurate and safe medication use
• Develop and implement pharmaceutical care plans to improve patient outcomes
• Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
• Participate in quality improvement initiatives to enhance patient safety and care

• Eligibility for licensure to practice pharmacy in the Commonwealth of Massachusetts
• Postgraduate residency/fellowship training preferred
• Board Certification by examination, or equivalent certification, in the relevant specialty preferred
• Strong knowledge and skills in clinical pharmacy practice, including medication therapy management and patient education
