Additionally, we have been awarded as a certified Great Place to Work for three years in a row and Modern Healthcare’s "Best Places to Work" Overview Signature HealthCARE is excited to offer a specialized training program for new Minimum Data Set (MDS) Nurses, designed to equip Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) with the skills and knowledge necessary to excel in long-term care
This program spans 8 weeks, with training sessions include in classroom and handoff at facility level
It provides a unique opportunity to develop expertise in MDS processes, ensuring high-quality care for residents while meeting regulatory requirements
Must have intermediate technology skills
Internal with no attendance write ups or performance concerns
We are searching for compassionate, detail-oriented, and driven nurses who are eager to make a difference in the lives of our residents
As an MDS Nurse, you will play a critical role in assessing and coordinating care, and this program is designed to prepare you for success in this vital position
Training Program Structure: Duration: 8 weeks Schedule: Training will be conducted in classroom and facility level
Content: The program will cover all aspects of the MDS process, including resident assessments, care planning, documentation, compliance with federal and state regulations, and interdisciplinary team collaboration
How you Will make a difference Key Responsibilities: Conduct Resident Assessments: Learn to accurately complete MDS assessments to capture the care needs of residents
Care Planning: Develop individualized care plans based on assessment data to ensure comprehensive care
Regulatory Compliance: Gain a deep understanding of MDS regulations and guidelines to maintain compliance with federal and state requirements
Interdisciplinary Collaboration: Work closely with other healthcare professionals to ensure holistic resident care
Documentation: Master the art of precise and timely documentation to support care delivery and regulatory compliance
Exhibit Competency: Demonstrate proficiency in all areas of the Resident Assessment Instrument (RAI), Care Planning, Prospective Payment System (PPS), Medicare, and, when applicable, Medicaid Case Mix
Program Outcomes: By the end of this program, participants will be able to: Assess Patients: Accurately assess new residents and evaluate the quality of care for existing residents, ensuring that each resident's needs are identified and addressed
Monitor and Document: Effectively collect, monitor, and document patient care information, utilizing this data to create comprehensive reports that guide care decisions and ensure compliance
Develop Care Plans: Create and implement individualized care plans that include both short-term and long-term goals for resident improvement, tailored to each resident's specific needs and conditions
Meet with Teams: Collaborate with caretakers, families, and multidisciplinary treatment teams, fostering communication and teamwork to provide the highest quality of care for residents
What you Need to make a Difference Qualifications: Licensure: Must hold a current and active LPN or RN license in the state of employment
Attention to Detail: Strong organizational skills and attention to detail are crucial for accurate MDS assessments and documentation
Customer Service Orientation: Must be able to provide excellent service to residents, families, and colleagues
Communication Skills: Effective communication with residents, families, and the interdisciplinary team is essential
Commitment to Learning: Willingness to engage fully in the 16-week training program and apply new knowledge in practice

Location Flexibility: While we strive to meet applicants' top three location preferences, please note that business needs may require placement in any of our facilities across the Kentucky, Tennessee, Indiana, and North Carolina
A willingness to potentially relocate is essential
