PRINCIPAL DUTIES AND RESPONSIBILITIES: Clinical: The APP will deliver Primary Care and Urgent Care to the practice's patients. Specifically, the APP will participate as a full member of the clinical team, collaborate with MD partners, work closely with other advanced practitioners, the medical support team, and administrative staff to deliver expert team-based care.Perform and record the patient history and physical exam; form an assessment and plan, which may include ordering appropriate testing and imaging, consulting with specialists, or discussing findings with a practice physician when appropriate.Follow and manage patients (in collaboration with MD), enhancing continuity of care.Act upon labs, imaging, and other studies promptly.Respond to patient questions and messages within the requisite time frame.Maintain and practice in accordance with practitioner guidelines per state and hospital regulations.Maintain and update clinical knowledge and skills based on current health care practice/research.Maintain the standards and educational requirements specific to the position and licensure. Administrative: Complete patient notes within the requisite time frame.Participate and contribute as a member of the advanced practitioner team.Serve as a resource to nurses, medical assistants, and staff for clinical questions/problems and participate in care when indicated.Promote mutual cooperation, support collegial relationships among all staff.Promote an environment that is sensitive to the culturally diverse needs of patients and employees.QUALIFICATIONS: Master's degree in nursing required. This position requires 5-7 years clinical experience as a Nurse Practitioner, preferably with expertise in primary care. A NP license is required.
