SUMMARY

The Psychiatric & Mental Health Nurse Practitioner (PMHNP) will focus on providing high quality, holistic patient care as part of an integrated healthcare team. Complexity level includes diagnosis and care of patients/clients with persistent to severe mental illness, medication management, addictions and addictive personalities, survivors of homelessness, domestic violence, sexual assault, and other traumas.

Works with the behavioral and mental health teams cross departmentally within Pillars Community Health. Collaborates with providers and Other Licensed Providers. Screening, Brief Intervention, & Referral to Treatment = SBIRT which is a large part of the position with children, adolescents, and adults. SBIRT is a federal grant program designed to link medical and behavioral care together.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1) Diagnose and treat acute, episodic, or chronic mental illnesses for patients across the lifespan. 2) Prescribe all necessary medications and treatments including Medication-Assisted Treatment and Medications for Opioid Use Disorder. (MAR/MOUD). 3) Perform comprehensive psychiatric evaluations of patients. 4) Perform routine follow up appointments for ongoing symptom and medication management. 5) Order and evaluate laboratory tests and other diagnostic tests as indicated and within the scope of practice. 6) Collaborate, consult and/or refer psychiatric patients to medical providers as clinically indicated for medical recommendations, further medical workup and continued medical management. 7) Develop and present for consideration/recommendation, ongoing patient medication management policies and procedures that coincide with the philosophy of evidence-based approach to total physiological and psychological healthcare. 8) Facilitate referrals to other healthcare professionals in and out of Pillars Community Health. 9) Communicate with collaborating/consulting physician and other specialists in providing tailored and ongoing continuum of care for each patient. 10) Responsible for performing the necessary functions of the integrated care position including screening, brief intervention, and referral to treatment (elements of an SBIRT model) in accordance with agency Scope of Practice Guidelines, practice certifications, agency privileging, protocols, and standing orders. 11) Participate in collaborative consultation with medical providers regarding mutual patients. Complete warm hand-offs as schedules permit. 12) Assist in brief intervention for patients including, but not limited to, utilizing motivational interviewing, behavioral and cognitive behavioral therapy techniques in context of appointments 13) Complete referrals to behavioral health team. Complete universal screenings or assessments of all psychiatric patients in the areas of depression, anxiety, substance abuse, domestic and sexual violence.

Patient Records and Data Collection 1) Ensure all patient records, charts, and all related documentation are current, maintained, and consistent with best practices in the health care field, in accordance with organizational policies and, as well as within all relevant laws and regulations. 2) Responsible for documentation and coding of patient care; records all interactions and scoring or screenings. 3) Maintains patient information and confidentiality in accordance with applicable laws and organizational policies. 4) Facilitate and coordinate data collection efforts in conjunction with the Behavioral Health integration Project Team as assigned. Care Coordination and Integration 1) Collaborate with other medical and behavioral health professionals to develop, implement, audit and modify procedures for screening, assessment, brief intervention, and referral to treatment. 2) Work collaboratively with onsite behavioral health team to manage the needs of patients. 3) Convene collaborative care meetings with the BH Team members (assigned PCH Therapists) to consult about patients and ensure continuity and maintaining appropriate level of care. Participate in care coordination meetings with medical, psychiatric, and behavioral health providers as scheduled. 4) Collaborate with Behavioral Health Team to facilitate the ongoing mission of integration of health services, and to carry out the goals and focus areas of the behavioral health integration grant along with the team.

Education/Experience Must be an Advanced-Practice Registered Nurse in the state of Illinois who has completed a master’s program in nursing (MSN) or doctoral degree program (DNP) from an accredited program. Holds a Psychiatric-Mental Health Nurse Practitioner-Board Certified (PMHNP – BC) credential. Previous experience in nursing or other medical fields. Prior experience working in a federally qualified health center preferred.

Language Bilingual (English/Spanish) skills preferred but not required for consideration.

Required Skills Ability to build rapport with patients. Ability to thrive in a fast-paced environment. Excellent written and verbal communication skills. Strong leadership qualities. Desire to work as a cohesive team member in a growing integrated healthcare setting.

Schedule Patient appointments are mostly scheduled during daytime clinic hours M-F between 8am and 5pm. Visits may be scheduled in-person or virtually to accommodate patient needs. May include (or require) some availability for evenings and at least one Saturday each month.

Other Position will require travel between PCH sites in local service area. Mileage for authorized work-related purposes is reimbursable.
