Job Summary:

Key Responsibilities:

  2. Care Coordination: Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure that patients receive appropriate and timely care.

Qualifications:
  1. Bachelor's degree in Nursing, Social Work, or a related field (Master's degree preferred).
  2. State licensure or certification as required (e.g., RN, LSW, LCSW).
  3. Minimum of 2-3 years of experience in case management, preferably in a hospital or healthcare setting.
  4. Strong clinical assessment skills and knowledge of medical terminology.
  5. Excellent communication and interpersonal skills.
  6. Ability to work independently and as part of a team.
  7. Proficiency in using electronic health record (EHR) systems and other healthcare software.
  8. Knowledge of healthcare regulations, including Medicare and Medicaid guidelines.
