Aging Care Coordinator

DescriptionThe Aging Care Coordinator supports clients and their families by providing comprehensive assessments, developing care plans, monitoring ongoing care, managing the Therapeutic Companion, communicating with and coaching the family, and coordinating the interdisciplinary team.The Aging Care Coordinator is the point person for the family and will act as the liaison between the client, family, and providers to ensure effective care. Aging Care Coordinator will meet clients in the community, remotely, or in the home setting to perform actions necessary to meet their needs and goals.Job Responsibilities:Completing initial evaluations and comprehensive assessments of the client and their environment that are done both in-person and virtualConducting psycho-social evaluations and assessing cognitive abilities and moodReviewing clinical history and contacting providersDeveloping a care plan with the client and identified supportsMaintaining communication with client contacts, including family, physicians, attorneys, financial offices, and other care supportsSupervising Therapeutic Companions and working collaboratively with other OPG team membersDocumenting all client interactions, outcomes, and progress toward goalsProviding consulting services to the client and family members around agingConducting family meetings to improve family dynamicsProviding coaching services to identified individualsCommunicating with outside treatment providers and coordinating careTraveling throughout the area for evaluations, home visits, and appointmentsAdvocating for clients based on individual medical, financial, and social needsHaving a basic understanding of the differences of Medicare, Medicaid, Private Insurance, and Long-Term Care policiesHaving a basic understanding of care managementAssisting clients in moving into senior living communitiesParticipating in company supervision and completing required company education materialsAdditional Responsibilities:Emergency support, on-call availability, and client transportation, as neededAdditional duties will be assigned as deemed appropriateQualifications:RN, LSW, LCSW, LICSW, OT, or PT in good standing preferred; Case Manager or Care Advocacy in the elder field with 5+ years of experience also considered4-5 years of experience in care management, home care, acute care, and/or a rehabilitation settingExperience with elder careExperience in behavioral health preferredExperience working closely with families & professional providersExcellent written and oral communication skillsCommitment to Diversity, Equity, Inclusion, and BelongingWe are committed to promoting an environment that celebrates diversity, equity, inclusion and belonging in all areas of our organization.
