The Health Liaison is an employee of Choices Coordinated Care Solutions (Choices) but part of the
Burmese Refugee Health Collaborative (BRHC) initiative serving Burmese Nationals living in Allen
County, Indiana. The Health Liaison will provide culturally competent service navigation and patient
follow-up for the Burmese Nationals referred from the designated BRHC health clinic. The Health
Liaison will serve individuals in their primary language or will ensure the use of interpretive services.
The incumbent will support and uphold a commitment to racial and ethnic equity and antiracism.

Essential Duties and Responsibilities
*Articulates the mission, vision and practices of Choices and the Burmese Refugee Health
Collaborative.
*Conducts an initial assessment of client needs, providing trauma-informed navigation and referral
services for consumers and all household members from the BRHC clinic ensuring emergent
needs are met the same day whenever possible.
*Manages crisis situations as they emerge, exercising sound judgement and finding a resolution
that will best meet the client's needs while maintaining safety for everyone involved.
*Maintains accurate and detailed navigation documentation.
*Adheres to the policies and protocols established by Choices and the BRHC.
*Communicates effectively with Choices Management and all BRHC partners.
*Ensures collection and timely submission of all necessary service data.
*Willingly completes other duties as assigned to advance the mission of Choices and the BRH

Qualifications
*Excellent knowledge of resources in and around Allen County.
*Bi-lingual in Burmese/English (any dialect) strongly preferred.
*Experience communicating via an interpreter preferred.
*Highly organized, and able to manage multiple priorities simultaneously.
*Ability to quickly establish rapport with resistant clientele.
*Ability to problem solve independently in difficult situations.

Pay: $21.00/hr, 24 hours a week (Tuesday, Thursday, and Friday)
