Job DetailsJob LocationInpatient Center - Johnson Creek, WIDescriptionRainbow Community Care is currently accepting applications for dedicated CNAs to work on a flex time basis at the inpatient center in Johnson Creek. Cross-training into home hospice is also available. The inpatient center CNA is responsible for assisting patients with activities of daily living, communicating changes in condition, and working collaboratively with team members. Essential Duties include, but are not limited to:Assists with ambulation, transferring, and changing position of patients. Assists the patient with personal grooming, toileting, incontinence care, bathing, skin care, nail care, dressing, transferring and walking. Assists with preparing meals and assists patients with eating and drinking. Maintains a clean, safe and comfortable environment. Performs light housekeeping duties as assigned.Reads, understands, and follows the plan of care. Maintains up-to date patient records and documents cares provided to the patient.Reports changes in patient condition according to Rainbow policies. Drives to and from other office/meeting locations as necessary. Other duties as assigned.Personal Qualifications:Ability to be flexible and be included in weekend and holiday rotation schedule. Ability to maintain healthy boundaries and maintain positive relationships with patients, families, and team members. Ability to be self-directed and handle stressful and emergency situations.Ability to be tactful and courteous in contact and communication with others.Ability to maintain patient, family, and staff confidentiality. Professional Qualifications:Valid Wisconsin driver's license and current auto insurance is required and must be maintained. Current Wisconsin Nursing Assistant Certification is required. Minimum of 6 months CNA experience preferred.Hospice experience preferred.
