Job DescriptionRegions Hospital's Care Ecosystem Program Coordinator (CEPC) is a core member of the Care Ecosystem study team. The Care Ecosystem program is funded by a grant from the National Institute on Aging (NIA) to implement and evaluate a telephone-based support, education, and care coordination intervention for patients with dementia and their caregivers. The goal is to improve the health and wellbeing of patients with dementia and their caregivers, as well as to decrease unnecessary medical costs. The Care Ecosystem Program Coordinator is the primary point of contact for patients and their families enrolled in the program. CEPCs work collaboratively with participants and under direct supervision and guidance of a multidisciplinary clinical team.Responsibilities include:Participate in initial and ongoing training about common issues in neurodegenerative diseases, aging, caregiving, advance care planning, and medication management.Maintain regular telephone (or e-mail) contact with participants.Attend weekly debriefing sessions with clinical team to review cases and discuss issues, problem solving strategies, resource needs, and communication techniques.Use integrated workflow management technology for care delivery and data collection.Participate in evaluation and improvement by contributing feedback during team meetings, surveys, and one-on-one meetings with supervisor.Work schedule: FTE 1.0 (40 hrs. per week) Day ShiftRequired Qualifications:Bachelor's degree in a related field.Sufficient experience and demonstrated skills to successfully perform the assigned duties and responsibilities.Preferred Qualifications:Master's degree in a related field OR a minimum of two years' work experience and a bachelor's degree in a related field.Experience with implementing programs. Experience with electronic medical records and/or other innovative technologies.
