Waycross, GA 31501The primary purpose of this position is to provide direct skilled nursing, intervention and supportive care activities in the patients' homes in accordance with Hospice Satilla's Guidelines. And, maintain Hospice Satilla as a Center of Excellence.Consistently demonstrates Hospice Satilla's interdisciplinary approach to patient care. This includes but is not limited to:Assessments, plans, implements and evaluates phases of nursing process.Provision of skilled nursing assessment and hospice/palliative care in order to maximize the comfort and health of patients and families consistent with Hospice Satilla's Guidelines.Initiate communication with attending physicians and other community resources to report changes and coordinate optimal care and use of resources for the patient/family. Obtain data on physical, psychosocial, social and spiritual factors that may influence patient/family.Maintain and update patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.Monitor physical care of the patient by instructing primary caregivers, volunteers, and employed caregivers.Respond to medical or emotional crisis and make home visit to provide direct care as appropriate.Demonstrate familiarity with guidelines of agency and rules and regulations of community, state and federal bodies, which relate to the delivery of hospice services. Actively contribute in patient care conferences.Accept other assignments as appropriateShall receive patient care plan for the appropriate discipline and carry out the program with minimal direct supervision. Director of Nursing shall provide direct Clinical Supervision. Executive Director shall provide non-clinical supervision as necessary.Provide phone triage and support to patient/families as needed and requested.Assist with the maintenance of medical records, assuring accuracy, completeness and compliance with licensing and certification regulations as requested.Aids in building and maintaining Hospice Satilla's reputation, as a hospice care provider of excellence by contributing as professional member of staff. This includes but is not limited to:Consistently demonstrating professional standards as outlined in the Hospice Satilla Associate Handbook and as communicated by Hospice Satilla management. Conducting to a positive work team by sharing information, using problem-solving methods and accepting new ideas, criticism or advice from others.Demonstrating innovative, productive and cost-effective use of non-patient care hours.Patient RightsDemonstrates working knowledge of patient's rights during interaction with patient.Maintain confidentiality of all patient care information.Monitor care to ensure that all patients are treated fairly, and with kindness, dignity and respect.Ensure that all care is provided in privacy.Be knowledgeable of the resident's rights and responsibilities, including the right to refuse treatment.Committee FunctionsServes on various committees of the company as appointed by the Executive Director and the Director of Nursing.Staff DevelopmentDevelops and participates in programs designed for in-service education, on the-job training, and orientation classes for newly assigned associates.Participates in creating and maintaining an atmosphere of warmth, personal interest, and positive emphasis with the agency staff.Administrative FunctionsAssume administrative responsibility as needed in the absence of office staff.Safety and SanitationDemonstrates safe working practices. This includes but is not limited to:Works along with other associates to ensure that established safety regulations are followed, including fire protection/prevention, smoking regulations, infection control, etc.Identifies and corrects safety hazards and/or notifies the Director of Nursing, so, that safety hazard shall be immediately remedied.Maintains work areas in safe and orderly fashion. Maintains equipment in safe working order.Ensures use of proper body mechanics during treatment of patients and everyday office work.Utilizes universal precautions, by wearing appropriate protective equipment as necessary.Working ConditionsIs involved with patient, family members, associates, visitors, government agencies/associates, etc., under all conditions/circumstances to assure that patient's needs are met.I subject to hostile and emotionally upset patients, family members.Works beyond normal working hours and on weekends and holidays when necessary.Is subject to callback during emergency conditions (e.g., severe weather, evacuation, post disaster, etc.)Communication with other staff members.Is subject to injury from fall, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke and other air contaminants. Is subject to exposure to infectious waste, disease, conditions, etc., including TB and the AIDS and Hepatitis B viruses.May be subject to the handling of and exposure to hazardous chemicals.Physical RequirementsShould be able to speak and write the English language in an understandable manner.Should be able to cope with the mental and emotional stress of the position.Should function independently, have flexibility, personal integrity, and the ability to work effectively with patients, associates and support agencies.Should be able to assist in the evacuations of patients during emergencies.
