Role Overview:

Alameda Health System is hiring The Nure Manager of Palative Care is responsible for leading and developing palliative care nursing across Alameda Health System, including managing current and recruiting additional Palliative Care Nurses to work across AHS, collaborating with the Palliative Care Director, Division Chief, and other AHS Palliative Care leaders to expand and improve services for seriously ill patients across AHS, and provide clinical care to AHS palliative care patients, initially focused in the palliative care clinic, and then in other settings as needed to achieve program goals.

DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: Following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.

Manages a staff of palliative care nurses, and is fully accountable for all personnel matters including hiring, training, performance evaluations, disciplinary actions, terminations and recommendations for promotions and merit increases.Supervises staff and manages employee performance; provides on-going performance feedback, addresses problems, orients and trains employees, verifies competency and identifies and suggests way to develop skills; monitors workflow.Confers with staff regularly regarding techniques used and related matters for the purpose of promoting and evaluating professional development and assuring quality of care, as well as reviewing work in progress.Conducts staff meetings and ensures palliative care nursing coverage across the system.Serves as subject matter expert on Palliative Care Nursing.Serves as a liaison between Palliative Care Department and Nursing Department.Leads development and coordination of serious illness education and quality improvement programs for nurses across AHS.Collaborates with nursing education and direct reports to teach regular Palliative Care Nursing Trainings.Creates and maintains policies and procedures for palliative care nursing and serious illness care for the palliative care team and across AHS.Collaboratively works with other leaders to identify needs and develop services to address them.Contributes to quality improvement across AHS in serious illness care.Assists in the development and coordination of patient education.Coordinates Palliative Care educational programs for hospital staff, patients and the community.Performs other administrative managerial duties including development of annual budget process and performing office management functions.Collaborates with other departments to create and sustain interdisciplinary team roles.Provides palliative care nursing to AHS palliative care patients, initially focused in the palliative care clinic, and in other settings as needed to achieve program goals.Performs nursing duties within the Palliative Care Department. Orients and backs up nurses on palliative care consult service.Supports patients and families in symptom management, education, and advance care planning.Supports planning for transition of patients from palliative care services to hospice care.Works collaboratively with the Palliative Care interdisciplinary team.

MINIMUM QUALIFICATIONS:Required Education: Graduate of an accredited nursing program and Bachelor’s Degree in Nursing.

Preferred Education: Master’s Degree in Nursing.

Required Experience: Palliative Care/Hospice Nursing experience.

Preferred Experience: Leading palliative care education and quality improvement programs, Supervising other nurses, Experience in an array of work settings: hospital, clinic, post-acute/skilled nursing, and community, Work in the safety net setting with diverse patient and family populations.

Required Licenses/Certification: Certification through the Hospice and Palliative Credentialing Center (Certified Hospice and Palliative Nurse or Advanced Certified Hospice and Palliative Nurse) within 12-months of employment

Required Licenses/Certifications: California Registered Nurse.

Preferred Licenses/Certification: Certification through the Hospice and Palliative Credentialing Center (Certified Hospice and Palliative Nurse or Advanced Certified Hospice and Palliative Nurse)

Preferred: Bi-lingual or multi-lingual capability, especially Spanish, Cantonese, Mandarin, Vietnamese, Tagalog, Russian
