Description:

Brigham and Womens Anticoagulation Management Service (AMS) is a pharmacist-run clinic that provides full anticoagulation management services for all oral anticoagulants including warfarin (Coumadin, Jantoven), dabigatran (Pradaxa), rivaroxaban (Xarelto), apixaban (Eliquis), and edoxaban (Savaysa). Brigham & Womens (BWH) AMS has been recognized as a Center of Excellence by the Anticoagulation Forum. Patients are referred to our service by a Brigham and Womens/Faulkner Hospital attending physician, usually the patients primary care provider or a specialist such as a cardiologist or hematologist for management of anticoagulation therapy. BWH AMS is a service dedicated to caring for patients who require anticoagulation. Pharmacists will provide full anticoagulation management including, but not limited to, prescribing anticoagulant medications and their required laboratory monitoring tests, while working closely with the referring doctor under a Collaborative Practice Agreement.

We provide the following services:

Selection of most appropriate oral anticoagulant therapy based on patient specific variables (including direct-oral anticoagulants and warfarin)

Thorough education on anticoagulation based on national patient safety goals

Assistance with getting your medication filled at the pharmacy

Ordering and following labs based on patient specific factors

Management of anticoagulants around procedures or surgeries

Assessment and tools to improve adherence

24/7 emergency pager for triaging bleeding and clotting concerns

Patient Navigator will report to Ambulatory Pharmacy Manager and be part of a core team that works closely together to provide exceptional patient care.

Principal Duties and Responsibilities

  1. Patient navigation assistant

    1. Contacts patients enrolled in BWH AMS
    2. Coordinate patient enrollment including obtaining patient medical history and laboratory data.
    3. Facilitate patient access to information provided by the program.
    4. Communicate recommendations to patients as provided by the CDTM pharmacist
  2. Assist and help coordinate program development
    1. Help with patient correspondence and scheduling
    2. Help with any administrative tasks as needed
  3. Teaching and Education
    1. Educate patients
    2. Work with the CDTM pharmacist to provide information and education to patients

  4. Other duties as requested.

Qualifications:

College degree preferred. Especially well-suited for this position might include individuals with background/major in Public Health, Health Education, Communications, Psychology, Clinical/Health Research. Masters in Public Health potentially beneficial but not required. Similarly, individuals with pharmacy background and training may also be well suited for this role given relevant issues related to drug therapy. Prior experience with clinical research or customer service beneficial. Individuals interested in future positions prior to medical school, public health also potentially well suited for this role.

Skills and Abilities Required

  1. Excellent organizational skills given coordination of multidisciplinary team, various logistical components, data acquisition.
  2. Strong communication skills, especially regarding support for patients and their questions, issues; empathy, ability to learn, adapt to program approaches.
  3. Comfortability with telephone-based workflow
  4. Established ability to work with, coordinate teams.
  5. Computer skills: working knowledge, familiarity with computer, software including use of email, Excel.
  6. Personal abilities/attributes: Positive attitude, creative problem solving, hardworking, flexible, energetic, cognizant of patient confidentiality, appropriate professional demeanor, especially in interacting with physician offices.
  7. Fluency in Spanish or other languages are desirable; and being proficient in utilizing interpreter services
  8. Occasionally may require hours outside normal working hours, such as Saturdays
  9. Comfortable expressing ideas and feedback
  10. Prior experience as a Navigator Assistant or equivalent 1 year experience
  11. Able to work independently
  12. Pharmacy Technician Background Preferred

Working Conditions

Work will be hybrid, must be able to work on-site and remote throughout the week as assigned and attend on-site training and meetings at 830 Boylston St Chestnut Hill
